If you complete your paperwork online, you don’t need to arrive early for your appointment. But if you do, help yourself to a complimentary beverage and have a seat in our comfortable waiting room — your provider will be with you shortly. The only thing you need to bring is yourself and your insurance card (if you plan to use it). You are welcome to bring any additional information or documentation you feel is appropriate to review with your provider. You are also welcome to bring along any other individuals you choose, but including others in the appointment is your decision.
